TERMS:As an adult, parent, or legal guardian, I give my consent for myself and or child(ren) to participate in any of the Tiger Strong NYC/ Active Studios NYC Programs. I recognize the possibility of injury in martial arts, stretching, fitness, yoga, dance, or any other Active Studios NYC programs. I hereby waive and release Active Studios NYC and any Officers, Employees, Agents, and Assigns from any and against all liability claims, including accidents or injury while on or about their premises. To the best of my knowledge, the participant is in good physical condition, and I have no knowledge of any physical condition, injury, or illness that would place the participant at risk in a martial arts or any other Active Studios NYC class. This Acknowledgment of Risk and Waiver of Liability has been read, understood completely, and signed voluntarily by an adult, parent, or legal guardian if a student is under 18 years of age.
PHOTOGRAPHY RELEASE:I hereby authorize Tiger Strong NYC/ Active Studios NYC, its representatives and members the right (with my explicit permission) to take photographs of me and/or my child/children to publish photographs and/or videos , to be use in any Tiger Strong NYC / Active Studios NYC printed publications and/or website. I acknowledge that since my and/or my child/children's participation in publications and/or websites produced byTiger Strong NYC / Active Studios NYC is voluntary, I/we will receive no financial compensation. I further agree that my and/or my child/childrens's participation in any publication and/or website produced by Active Studios NYC confers upon me and/or my child/children no rights of ownership whatsoever. I agree that Tiger Strong NYC / Active Studios NYC may use such photographs and/or videos of me and/or my child/children with or without my and/or my child/children's name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and Web content. I release Tiger Strong NYC /Active Studios NYC, its contractors and its members from liability for any claims by me, my child/children or any third party in connection with my and/or my child/children's participation.
